Mounjaro vs Wegovy: Clinical Comparison

Tirzepatide and semaglutide are both prescription, once-weekly injectable medications used in medically supervised weight management programmes. They are indicated to support appetite regulation, improve metabolic parameters, and facilitate gradual weight loss when combined with appropriate lifestyle interventions.

Mechanism of Action

Semaglutide, marketed as Wegovy, is a GLP-1 receptor agonist. It works by mimicking the action of the endogenous hormone GLP-1, leading to increased satiety, delayed gastric emptying, and reduced appetite. This contributes to lower caloric intake and sustained weight loss over time.

Tirzepatide, marketed as Mounjaro, has a dual mechanism of action. It acts as both a GLP-1 and GIP receptor agonist, targeting two incretin pathways involved in glucose regulation and appetite control. This dual receptor activity may enhance effects on insulin sensitivity, appetite suppression, and energy metabolism.

Clinical Effectiveness

Both medications have demonstrated significant efficacy in clinical trials.

Data from studies including the SURMOUNT-5 trial indicate that tirzepatide is associated with greater average reductions in body weight compared to semaglutide. Mean weight loss with tirzepatide has been reported at approximately 20% of baseline body weight, compared to around 13–14% with semaglutide. Greater reductions in waist circumference have also been observed in head-to-head comparisons.

However, clinical outcomes vary between individuals. Factors influencing response include adherence to treatment, baseline metabolic health, dietary habits, physical activity levels, and tolerability.

Individualised Treatment Selection

Despite differences in efficacy, neither medication is universally superior.

Treatment choice should be individualised and based on a comprehensive clinical assessment. Considerations include patient medical history, comorbidities, treatment goals, and potential side effects. Ongoing monitoring is essential to ensure both safety and effectiveness.

Role Within Weight Management

Both tirzepatide and semaglutide should be used as part of a broader weight management strategy. Lifestyle modification, including dietary changes and physical activity, remains a fundamental component of treatment.

These medications are not intended as standalone solutions but as adjuncts to support sustained behavioural change and long-term weight management.

Summary

Mounjaro and Wegovy are both effective pharmacological options for weight management. Tirzepatide may offer greater average weight loss in some patients due to its dual mechanism of action, but both treatments demonstrate clinically meaningful outcomes. Appropriate selection and clinical supervision are essential to optimise results and ensure safe use.
