Foundayo Once-daily Weight Loss Tablet
(Orforglipron)

Foundayo is the brand name for orforglipron, a prescription-only GLP-1 receptor agonist made by Eli Lilly, in the same class as Mounjaro and Wegovy. Unlike those, it's a once-daily tablet, not an injection. It reduces appetite and helps you feel fuller for longer, alongside a reduced-calorie diet and increased activity.

Most GLP-1 medicines are peptide-based and broken down by digestion, which is why they're injected, and why oral options like Rybelsus need strict fasting rules. Orforglipron is a small, non-peptide molecule that survives digestion without an enhancer, so it can be taken any time, with or without food.

Adults with a BMI of 30+, or 27 to 30 with a weight-related condition such as high blood pressure, type 2 diabetes, PCOS or sleep apnoea, and adults with type 2 diabetes needing better blood sugar control.

Started low and increased gradually, with a minimum of 30 days per strength: 0.8mg, 2.5mg, 5.5mg, 9mg, 14.5mg, 17.2mg (maximum). Not everyone needs the top dose, some people do well at 5.5mg or 9mg and stay there.

Both are once-daily oral GLP-1 medicines, but they're built differently. Wegovy pill is a peptide that relies on an absorption enhancer called SNAC, and even with strict fasting rules (empty stomach, no more than 120ml of water, a 30-minute wait before food, drink or other medication), the body only absorbs around 1% of each dose. Foundayo is a small, non-peptide molecule the gut absorbs directly, roughly 79% of each dose, with no enhancer and no fasting rules needed.

On weight loss, Foundayo's highest dose produced an average 11.1% weight loss over 72 weeks in ATTAIN-1, compared with around 14% over 64 weeks for Wegovy pill's highest dose in its OASIS 4 trial. These are separate trials with different populations and durations, so they aren't a direct head-to-head. Both are room-temperature tablets, so the practical difference comes down to Foundayo not requiring fasting or timed dosing around meals.

Mainly gut-related, easing as the dose increases gradually. Common: nausea (worst in the first 4 to 8 weeks or after a dose increase), stomach pain, constipation, diarrhoea, bloating, vomiting, headache, tiredness, and sometimes heartburn, indigestion or hair loss. Serious (seek urgent help): pancreatitis, severe stomach problems, dehydration or kidney problems, low blood sugar, serious allergic reaction, gallbladder problems, vision changes.

Anyone with a personal or family history of medullary thyroid carcinoma or MEN 2, severe liver impairment, or a known allergy to orforglipron. Tell your prescriber if you have a history of pancreatitis, kidney problems, delayed stomach emptying, diabetic eye disease, or upcoming surgery. Not recommended in pregnancy or breastfeeding; reliable contraception is advised, and hormonal contraception may need backup around dose increases, as Foundayo can affect absorption of other oral medicines.
